ALL YOU NEED TO KNOW ABOUT THE INDUSTRIAL ATTACHMENT

ATTACHMENT UPDATES FROM ADONGO DANIEL ADOMBILA (FASSA PRESIDENT ELECT) AND LIVINGSTON APEM NYARKO(FBNE PRESIDENT ELECT)

Before you read this long guideline 🤣🤣🤣🤣🤣, please take this is our advice for you,
Dont soil or dirty your log book.
Dont use correction fluid inside your log book
Try and carry a Book(jotter) along so that you can take notes of what you will be learning if possible.
It will be good if you can always write what you learn during the day in you Jotter then after work you sit down and summarize it in your log book.
Dress Neatly and Appropriately to work and be there on time
Try and take pictures on your first day of Attachment if possible for future memories🤣🤣🤣🤣🤣
As the attachment is ongoing, through out the whole period, try and get pictures of yourself actively partaking in activities, it will be needed during your report writing.

*REMEMBER THERE IS A SAYING WHEN YOU GO TO ROME DO WHAT THE ROMANS DO, ABIDE BY EVERY RULE THAT GOVERNS THE ORGANISATION YOU WILL FIND YOURSELF*

*FILLING OF LOG BOOK*

1. Make sure you Fix a recent *PASSPORT PICTURE* of yourself in the space provided in Page 1 of the log book *(COMPULSORY).*

2. *Students Registration Number* means your *school index number* in full.
Eg BC/HMT/21/000 OR 0718000000.

3.Make sure you write you name in full in the space provided,*SURNAME* first then your other names.

4.*STATUS* means are you a regular student or an evening student. If you attend lectures in the *MORNING* you are supposed to *TICK* *REGULAR* and if you attend Lectures in the *EVENING* you are to tick *EVENING*

5. *YEAR Of STUDENT* Means what is your current level in school, you finishing the academic year doesn’t mean you have moved 1 level ahead already, the attachment is for the level you are in now, take note of that. if you are in level 100 now *DONT* tick 200 because you think you are now going to level 200, the same for the other levels too

6. Write your *FACULTY* name in full, e.g
FACULTY OF APPLIED SCIENCES,
*Or*
FACULTY OF APPLIED ARTS AND TECHNOLOGY,
*Or*
FACULTY OF BUSINESS STUDIES,
*Or*
FACULTY OF ENGINEERING,
*Or*
FACULTY OF BUILT AND NATURAL ENVIRONMENT.

7. Write your *DEPARTMENT* Name in full

8. Write you Program in Full *EXAMPLE*
HND IN INFORMATION TECHNOLOGY
*OR*
BACHELOR OF TECHNOLOGY IN MARKETING AND STRATEGY

9. Write in full the *HOUSE ADDRESS* you will be staying while embarking on the attachment.

10. Write you *Mobile Phone* Number (s)

11. Duration of Attachment, you are supposed to write *FROM 15TH AUGUST TO 30TH SEPTEMBER* just as stipulated in your various *ATTACHMENT* letters

12. You write the *DATE* you actually started the *ATTACHMENT* then you sign.

*PAGE TWO OF THE LOG BOOK*

1. Write the *FULL NAME* of the COMPANY OR ORGANIZATION.

2. Enquire about their *TELEPHONE NUMBER* and write it down.

3. Enquire about their full *POSTAL ADDRESS* and write it down.

4. *EXACT LOCATION* write the exact location of the company indicating a PROMINENT LANDMARK or popular place nearer to the company, if you don’t know please ask the company dont go about stressing yourself 😂😂

5. NATURE OF BUSINESS what exact business or activities is the company engaging in, or what are the SERVICES they provide

6. AREA OF SPECIALIZATION some big companies engages in a lot of activities but actually specialise in certain aspects, please ask them they will tell you. REMEMBER to fill the YEAR the company or organization started.

PAGE THREE OF LOG BOOK

You will be starting with WEEK ONE so Indicate it at the TOP RIGHT CORNER OF THE LOG BOOK.

THEN YOU begin write the various dates according to their days in the month.
E.g 15/08/2022 in that order.

At the end of the week, you are requuired to give your remarks about the whole learning process in the week and the section or department you were during the week. You sign and also let your supervisor sign and certify it for you.

WHAT IS AN ASSUMPTION OF DUTY LETTER AND HOW IS IT IMPORTANT IN THE ATTACHMENT PROCESS

ASSUMPTION OF DUTY LETTER refers to a letter or form which is supposed to be filled by every student undertaking INDUSTRIAL ATTACHMENT, it should and must be filled from your PERSONAL STUDENTS PORTAL. It must be done and submitted within 7 days after the student starts his or her attachment
It is this letter that will inform AUTHORITIES about where you are doing your INDUSTRIAL ATTACHMENT so that a SCHOOL BASED SUPERVISOR can be assigned to you. It will enable the SUPERVISOR to be able to come around and SUPERVISE you when it is time for SUPERVISION.

It is therefore prudent you take part in it

HOW TO FILL AN ASSUMPTION OF DUTY LETTER OR FORM ONLINE

IPHONE USERS ARE ADVISED TO YOU ANDROID PHONE IN DOING IT TO BE ABLE TO GENERATE THE REQUIRED PDF AFTER SUBMITTING

Take note:
ASK OF EVERY INFORMATION NEEDED FROM THE COMPANY

1. Kindly log onto your students portal using the link records.ttuportal.com

2. Click on MENU at the TOP LEFT CORNER of your phone screen. Scroll down and click on ASSUMPTION OF DUTY FORM

3. Fill the Correct Deatils of the company as being requested by the form.
4. Remember to select your zone of Attachment for the default one is Western Region. So make sure you CLICK AND SELECT the ZONE where you find yourself during this ATTACHMENT PROCESS

5. Letter Addressed to Should be selected based on who is in charge of the Organisation.

6. Date you started the Attachment and the Company supervisor Phone NUmber

7 Click on *NEXT* and accept the *Terms and conditions* then *CLICK ON SUBMIT*

8. *WAIT PATIENTLY* for the *FORM* to load and *GENERATE* a *PDF* for you, save it and print it out.

9. Give it to the Company for OFFICIAL SIGNING and STAMP(IF ANY)

10. Make sure you SIGN your portion of the letter and the date too.

11. FOR OFFICE USE Under no circumstance should you fill this section or allow your company based supervisor to fill it, leave that section blank, It is not supposed to be filled by you or your supervisor.

*SUBMISSION OF THE ENDORSED LETTER*

You are reuired to *SUBMIT* it at the *INDUSTRIAL LIAISON OFFICE*

Those of you not within Takoradi,  Kindly exercise Patience, we will verify and get back to you by close of work on MONDAY about how you will submit yours.

BEAR IN MIND

THE ASSUMPTION OF DUTY FORM IS SUPPOSED TO BE SUBMITED WITHIN 7 DAYS AFTER YOU START ATTACHMENT, BUT NOT AFTER 7 DAYS GET IT CLEAR
